This website is operated by the Southern Vermont Area Health Education Center, Inc. (SVAHEC), and the Champlain Valley Area Health Education Center (CVAHEC), both Vermont non-profit corporations. The Area Health Education Center, Inc. is sometimes referred to as "we" or "us." We appreciate the importance of safeguarding personal information and are committed to protecting the privacy of information that you may submit to us. This privacy policy ("Policy") is offered to answer the following questions:

    The Policy covers those web pages with a "ptophelp.org" Internet address (the Internet address is typically displayed at the top of your browser).
  2. Who Does This Policy Apply To?
    This Policy is provided for the benefit of the users of ptophelp.org. Since ptophelp.org is an online directory that makes its member information public, this policy does not apply to information submitted through the ptophelp.org web site by members of ptophelp.org or by SVAHEC or CVAHEC with the member's consent. This Policy does not apply either to information that may be collected by SVAHEC or CVAHEC offline.
  3. What Personal Information Do We Collect?
    You may use this web site to locate health care practitioners without providing any personal information (e.g., your name, address, or phone number). We only obtain personal information if you choose to provide it to us. However, we may collect personal information if you submit, through the web site, a question to us: For example, if you submit a question to us by email through the "Contact Us" page which you may access from our home page.
  4. How Do We Use Personal Information?
    No Sales of Personal Information. We do not sell any personal information.

    Questions. You may, via e-mail, mail, phone or fax, submit an inquiry to us. The inquiry may contain your e-mail personal identifiers such as your email address or phone number. We may, depending on the context, respond to your inquiry and we may retain your inquiry in our files.

  5. Can Personal Information Be Removed From Our Files?
    You can have us remove personal information from our files by sending an e-mail to us at info@ptophelp.org. In the e-mail, please provide your name and identify the specific activity in which you provided the personal information. We will seek to honor your requests in all instances where we are reasonably able to do so.
  6. Do We Use Cookies And If So, For What Purpose?
    "Cookies" are electronic pieces of information which can be stored by your browser on the hard drive of your computer. We use cookies to allow members to enter their user name and password less frequently during a session. Session cookies are automatically deleted from your hard drive at the end of a session or when the browser is closed. We also use cookies to store your consent to the User Agreement so that your consent is only required once. These cookies are saved to your hard drive and do not expire.

    Most browsers automatically accept a cookie, but you can usually modify the settings on your browser to refuse cookies or to let you know when one has been accepted. In Netscape Navigator, you may change cookie settings by selecting Edit on the menu bar, then Preferences, Privacy & Security and Cookies. In Internet Explorer, select Tools on the menu bar, then Internet Options, Security and Custom Level. Please note that these instructions vary depending on the version of the browser.   7. Do We Track Your Use of the Web Site?
    After a member successfully logs in, we only track the type of browser the member is using, the date the member logged in and the total number of times the member logged in. We do not track use of the web site if a user does not log in.
  8. What About E-mails You Submit Through the Web Site?
    You may submit e-mails through the web site, and those e-mails will often contain your personal information. This Policy applies to e-mails you submit to us. The Policy does not apply to personal information you may include in e-mails that you submit to anyone other than us, such as health care practitioners whom you have located through use of the website. By using our web site, you consent to the Policy. We will post changes to the Policy on this page so that you can always be aware of the types of information that we collect and how we use that information.
